ONLINE YOGA SUBSCRIPTION RELEASE & WAIVER

I hereby acknowledge that fully understand that by checking the box below I agree to the following terms:

Yoga Services Provided by 1000503326 Ontario Inc.:
1. That all online Yoga Services and related services / instructions are provided by 1000503326
Ontario Inc. (hereinafter referred to as the “Corporation”) and not by Leandra Antonutti in her
personal capacity.

Risks Associated With Your Participation:
2. You acknowledge that there are risks associated with your participation in the Yoga Services and
related services / instructions provided by the Corporation, including personal injury, and that by
participating you accept said risks at your peril.
3. You acknowledge and understand that you are free to withdraw or reduce your participation in the
Yoga Services and related services / instructions at any time.
4. You have taken the necessary steps, including seeking medical advice, to determine that you do
not have any medical conditions which prevent you from participating in the Yoga Services and
related services / instructions prior to you subscribing. You further acknowledge that should you
injure yourself or acquire any medical condition during your subscription it is your responsibility
to withdraw from your subscription.

Non-Recording of Online Classes:
5. The Corporation expressly forbids any online recording or retransmission of any of its Yoga
Services or related services / instructions. You acknowledge and agree that any type of recording
or transmission (including by not limited to video, auto, still photography, streaming, social media
posting, etc.) of any Yoga Services and / or related services / instruction, whether online or in
person, is strictly prohibited. This prohibition also extends to any classes provided through online
platforms including but not limited to SnapChat, Facebook, Instagram or Zoom etc.
6. You further acknowledge that any violation of the Non-Recording of Online Classes policy is
grounds for immediate exclusion from the Yoga Services and related services / instruction and
that violation of this policy shall cause damages to the Corporation for which you will be
responsible for, including any associated legal fees with the enforcement of said policy.
7. You further agree to indemnify, defend and hold harmless the Corporation, its Directors, Officers,
Employees, Agents and Instructors from any and all claims, lawsuits or other actions.

Release and Waiver:
8. In consideration of the acceptance on your online subscription you hereby agree to forever release
the Corporation (including its Directors, Officers, Employees, Agents and Instructors) and
Leandra Antonutti (including her Heirs, Executors and Administrators) from any and all claims or
personal liability or loss from personal injury, loss of life, property damage or any other damages
you may suffer as a result of your participation arising from or related to your participation in the
Yoga Services and / or related services / instruction. This release extends to any claims currently
known or may become known in the future, including any claims related to negligence, breach of
statutory duty or any other duty (including any duty owed under the Occupier’s Liability Act).
You agree that this Release and Waiver is intended to be as broad and inclusive as permitted by
law. Any provision found to be invalid or unenforceable by a Court shall not affect the validity or
enforceability of any other provision.
9. In the event you commence any claim, complaint or take any proceeding or threaten to make any
claim, complain against the Corporation and/or Leandra Antonutti, such claim, complaint or
proceeding will have been commenced in bad faith and this document may be raised as an
estoppel and as a complete bar to any such claim, complaint or proceeding.

Governing Law and Attornment:
10. You acknowledge and agree that this Release and Waiver is governed by the laws of Ontario and
that you attorn to the jurisdiction of Ontario, regardless of your location.

Your Right to Independent Legal Advice
11. By clicking the box below you confirm that you are over the age of 18, have read, understood and
agreed to abide with the terms of the above and that you have the right to have this Release and
Waiver reviewed by a lawyer prior to your clicking the box below.
